Quick answer

Is it worth going to Costa Rica?

A Brazilian butt lift in Costa Rica commonly costs from about 4,000 US dollars, often well over half below typical US pricing. Costa Rica is an established cosmetic surgery destination with many surgeons trained internationally. But weigh the saving carefully: a BBL carries the highest mortality of any cosmetic operation, mostly from fat being injected too deep, so the surgeon's technique and the facility matter far more than the price.

Costs below are indicative ranges, reviewed November 2025, and vary by the amount of liposuction and fat transferred, the technique, and what a package includes. They are not a quote and not a promise of a result.

What it really costs here

The price in Costa Rica, honestly.

In Costa Rica a BBL commonly runs from about 4,000 to 6,000 US dollars, which is roughly £3,200 to £4,700. The procedure usually combines liposuction to harvest fat with transfer to the buttocks, so packages often bundle both. Price is driven by the volume of liposuction, the technique and the facility.

Indicative ranges, reviewed November 2025. Not a quote. Because Costa Rica prices in US dollars, exchange surprises are unlikely. With a BBL especially, a price far below the market is a reason to ask hard questions about safety, not a bargain.

The realistic experience

The trip, recovery and the follow up problem.

Costa Rica is close to North America, but a BBL has a demanding recovery. You cannot sit directly on the buttocks for weeks, which makes the flight home awkward, and the clot risk from surgery plus a flight is real. Expect a compression garment, a special cushion for sitting, and a shape that settles only after some of the transferred fat is reabsorbed.

01

Days 1 to 3

Surgery combines liposuction with fat transfer, usually under general anaesthetic. Expect soreness at both the donor sites and the buttocks, a compression garment, and strict no sitting instructions.

02

Week 1 to 2

You sleep on your front or side and use a special cushion to avoid pressure on the new fat. Gentle walking lowers clot risk. Reviews happen in this window before travel.

03

Day 14 to 21

Surgeons commonly advise waiting two to three weeks before flying, and the sitting restriction can run longer. Confirm your own clearance to fly directly with the operating surgeon.

04

3 to 6 months

Some of the transferred fat is reabsorbed and the shape settles. The final volume is always less than on the day, and no surgeon can promise an exact size.

The follow up problem

If a fat collection, an infection or an uneven result appears once you are home, it is hard to manage from abroad, and local surgeons are often reluctant to take on another clinic's work. Agree before you travel how reviews and any revision are handled.

Insurance and complication cover

Standard travel insurance rarely covers elective cosmetic surgery or its complications. Read the policy closely and consider dedicated medical complications cover before you book.

The risks of travelling here

What can go wrong.

A BBL is not a routine procedure. It has the highest death rate in cosmetic surgery, and travel adds further risk. Read these honestly before you commit, wherever you go.

Fat embolism, the most serious risk

If fat is injected into or below the muscle, it can enter a vein and travel to the lungs, which can be fatal. Safe practice means injecting only above the muscle, ideally with ultrasound guidance. This is the single biggest reason technique matters.

Clots from flying after surgery

Liposuction and a flight both raise the risk of deep vein thrombosis and pulmonary embolism. Allow real recovery time and get written clearance to fly, even though Costa Rica is a shorter trip from North America.

Fat reabsorption and uneven result

Not all transferred fat survives, so the buttocks shrink from the early size, and the result can be asymmetric or lumpy. A touch up is sometimes needed, and the final shape is never guaranteed.

Infection and fat necrosis

Infection, fluid collections and areas of fat that do not survive can appear days later, sometimes once you are home, and may need treatment.

The no sitting recovery

You cannot sit normally for weeks, which affects the flight home, work and daily life. People who underestimate this often regret rushing the trip.

How to choose safely in Costa Rica

Five questions before you pay.

We do not name clinics. Use these to judge any clinic in Costa Rica you are sent to. With a BBL, treat any answer that downplays the fat embolism risk as a reason to walk away.

1
Is the surgeon a board certified plastic surgeon?
Ask whether the surgeon is registered with the Costa Rican College of Physicians and Surgeons and certified in plastic surgery, ideally with membership of the national plastic surgery association. Verify the registration directly.
2
Is fat injected only above the muscle, with ultrasound?
This is the question that matters most. Safe technique means staying above the muscle, and ultrasound guidance helps confirm it. A clear, confident answer is essential.
3
Is the facility a licensed surgical hospital?
A BBL belongs in a licensed facility with anaesthetic support and monitoring. Ask about the hospital or surgical centre, and any international accreditation it holds.
4
How many BBLs are done in one day?
Be wary of high volume operations running many cases a day. A careful surgeon limits numbers so each case gets full attention and proper monitoring.
5
What does the package include, and what about home follow up?
Get the inclusions, garment, cushion, reviews and revision terms in writing, including who pays and how a revision works once you are back home.

Common questions

The things everyone asks.

Is a BBL in Costa Rica safe?

A BBL is the highest risk common cosmetic procedure anywhere, mostly from fat injected too deep. At a licensed facility with a board certified plastic surgeon using safe technique and ideally ultrasound, the risk is much lower. Location matters less than the surgeon. This page helps you tell a careful clinic from a risky one.

Why is a BBL so much riskier than other procedures?

The danger is fat embolism: if fat is injected into or below the buttock muscle it can enter a vein and reach the lungs. Injecting only above the muscle, with ultrasound guidance, is what makes it safer, which is why technique is the whole question.

How long should I stay in Costa Rica after surgery?

Plan for roughly two to three weeks. Even though flights to North America are short, the clot risk is real and you cannot sit normally for a while, which makes flying awkward. Confirm your own clearance with the operating surgeon.

Will the result match what I see on the day?

No. Some of the transferred fat is reabsorbed over three to six months, so the final size is smaller than the early result. A touch up is sometimes needed, and no surgeon can promise an exact shape.

Should a very low price worry me?

For a BBL, yes. A price far below the market can point to a high volume operation or corners cut on safety, which is exactly the wrong place to economise given the risks. Choose on technique and facility, not headline price.
