What can go wrong.

SMP is generally considered low risk when done hygienically by an experienced artist, and serious complications are uncommon. The risks below are real and worth understanding before you commit, and most of them are about the look, not your health.

Common and usually temporary

Redness, mild swelling, tenderness, and tiny scabs over the treated area for the first few days. These settle on their own as the scalp heals.

Infection, which is uncommon

Because the skin is broken, there is a small infection risk if equipment is not sterile. Single use needles, a clean setup, and good aftercare keep this low.

Allergic reaction to the pigment, which is rare

A reaction to the pigment is uncommon but can cause redness, swelling, or itching. A patch test is sensible if you have reacted to tattoos, dyes, or cosmetics before.

Colour that shifts to blue or green

If the wrong pigment is used or it is placed too deeply, the dots can blur, migrate, or fade to a blue or green tone over time. This is a technique problem, and correcting it is slow and not always complete.

An unnatural hairline, the commonest regret

A hairline that is too low, too sharp, or the wrong shape is what people regret most. It traces to inexperienced work rather than to the method itself, which is why the artist matters so much.

A note on scans

There are occasional reports of tingling or swelling at tattooed areas during an MRI scan. The risk is considered low, but it is sensible to mention any scalp tattoo to the radiographer beforehand.

Recovery

What recovery actually looks like.

First 2 to 5 days

Redness and tenderness, with tiny scabs forming. Keep the scalp dry and avoid sweating, which means no gym or hot showers over the area for now.

Days 5 to 10

Mild flaking as the surface settles, and the colour looks lighter than on the day. Do not pick or scratch, which can lift pigment unevenly.

Weeks 1 to 4

Avoid sun, swimming, saunas, steam, and heavy sweating for about a month. Sessions are usually spaced about a week apart to build the density in layers.

After healing

Once fully healed, protect the scalp from sun with sunscreen or a hat to slow fading. Expect a touch up every three to six years to keep it sharp.

The risk specific to travelling

SMP is staged over several sessions, so travelling can rush the schedule. The industry is lightly regulated, and if the colour heals wrong, a correction is far harder to arrange from another country.

Why does SMP sometimes turn blue or green?

How soon can I exercise or swim?
