It treats short sightedness and some astigmatism, but unlike LASIK it is not generally used for long sightedness, so it does not suit everyone.

The real experience

Recovery, and the follow up.

SMILE is a quick outpatient treatment with the eye numbed by drops. You are awake, and the laser takes only a short time per eye. The laser shapes a thin disc of tissue inside the cornea, and the surgeon removes it through a small keyhole opening. Because no flap is created, the surface of the cornea stays largely intact, which is the basis of its appeal.

Vision is blurry and the eye can feel gritty and watery for the first day or so, then sharpens over the following days, with most people seeing well within a few days and the result settling over a few weeks. Many find the early recovery more comfortable than PRK. You use drops for a period and avoid rubbing the eye, swimming and dusty environments early on.

The follow up is the part medical travel makes harder. SMILE needs a check the day after and again as vision settles. If the result needs fine tuning, a further correction is less straightforward than with flap based surgery and may be done by surface laser instead. If you have surgery abroad, plan the early checks or agree who reviews you at home, keep the treatment notes, and arrange local eye care. Standard travel insurance rarely covers planned surgery or its complications.

The result can land slightly off target, so you may still need glasses for some tasks. A further correction after SMILE is less simple than after LASIK.

Rare surgical difficulties and glare

Rarely the tissue disc can be hard to remove cleanly, and some people notice glare or halos at night. Serious complications are uncommon but can affect vision.

The travelling risk

Laser surgery needs early checks and any problem needs to be seen quickly. Flying home too soon makes this harder. Agree the follow up and any further correction terms in writing, and arrange local eye care first.
