The one honest thing
Buccal fat removal is permanent, and the fat does not grow back. Taken too far, or done young, it can make the face look gaunt as you age.
Quick answer

Is Czechia worth it?

Buccal fat removal, sometimes called a bichectomy, in Czechia commonly runs from about €1,200 to €2,200 as an indicative range, well below typical UK private prices, which is the main reason people travel. Prague and other Czech cities are an established cosmetic surgery destination. The result depends far more on careful, conservative judgement than on the price, because the change is permanent.

Costs below are indicative ranges, reviewed December 2025, and vary with the anaesthetic, whether it is combined with other surgery, and what a package includes. They are not a quote and not a promise of a result.

What it really costs

The price in Czechia, honestly.

Buccal fat removal slims the lower cheeks by taking out the buccal fat pads through small incisions inside the mouth, leaving no visible scars. It suits a narrow group of people with full lower cheeks and good skin, and it is often oversold to people it will not flatter. A careful surgeon removes a conservative amount, because the change cannot be undone.

What drives the price is the anaesthetic, the surgeon's experience, whether it is combined with other facial surgery, and whether the quote is a true all in figure. The Czech Republic is cheaper than the UK mainly because overheads and staffing costs are lower, not because the surgery is lesser. The saving is real, but it is not the reason to choose a surgeon.

Confirm in writing what the quote includes, whether the anaesthetic, medication and follow up are covered, and the currency you pay in. Because the result is permanent, the surgeon's restraint matters far more than a few hundred euros.

The real experience

Recovery, and the follow up.

Buccal fat removal is a short day case, often under local anaesthetic with sedation, taking around half an hour to an hour. The surgeon makes a small incision inside each cheek, eases out a measured amount of the buccal fat pad, and closes with dissolving stitches. You go home the same day with some swelling and usually a soft diet for a few days.

Swelling inside and outside the cheeks is normal for the first couple of weeks and can briefly make the face look fuller before it slims. Most of the swelling settles over two to three weeks, but the final contour can take three to six months to show as the tissues settle. You keep the mouth clean with rinses and avoid hard or hot foods early on.

The follow up is what medical travel makes harder. The procedure needs a wound check and a look for any infection in the first weeks, and the result is only clear months later. If you have surgery abroad, agree who checks the healing once you are home and keep the operation notes. Standard travel insurance rarely covers planned surgery or its complications, so read the policy first.

The risks of travelling here

What can go wrong.

The surgery itself is small, but the decision is not, because removed fat does not come back. Czech clinics are well established, yet the biggest risk here is choosing the procedure when it does not suit you.

Common and usually temporary

Swelling, bruising, tightness and mild difficulty opening the mouth wide are normal in the first weeks and usually settle as healing progresses.

Over removal and ageing

Taking out too much fat, or doing this in a young face, can lead to a hollow, gaunt look that worsens with age as the face naturally loses volume. This cannot be reversed.

Asymmetry

The two cheeks may not match perfectly, and evening them out can be difficult once the fat is gone.

Infection and nerve or duct injury

Because the incisions are inside the mouth there is a risk of infection, and rarely the facial nerve branches or the salivary duct can be injured, which needs prompt specialist care.

The travelling risk

Any infection or healing problem needs to be seen quickly. Flying home soon after surgery makes early checks harder. Agree the follow up and any revision terms in writing, and arrange local care before you travel.

How to choose safely here

Questions before you pay.

A safe clinic answers all of these in writing without flinching. Vague answers, pressure to decide quickly, and no clear follow up plan are the warning signs that matter most. We never name a clinic, so use these to judge any you are sent.

1
Is the surgeon a qualified plastic or facial surgeon?
In Czechia ask whether the surgeon is registered with the Czech Medical Chamber and trained in facial plastic surgery. Ask how many of these procedures they do.
2
Am I actually a good candidate?
Buccal fat removal flatters only some faces and can age others badly. Ask the surgeon honestly whether your face suits it, and be wary of anyone who says yes to everyone.
3
How much fat will you remove, and why?
A conservative amount is safer because the change is permanent. Ask how the surgeon decides how much to take and what they would not do.
4
What does the quote actually include?
Get an itemised quote covering the surgeon, anaesthetic, facility, medication and follow up visits, with the currency stated.
5
How is follow up handled once I go home?
Confirm who I contact for problems and how records are shared so a doctor at home can check healing or manage any issue.
6
What happens if I am unhappy or something goes wrong?
Get the plan for complications in writing. Because removed fat cannot be replaced, ask frankly what can and cannot be corrected.

Common questions

The things everyone asks.

Is buccal fat removal in Czechia safe?

Czech cosmetic surgery is well established and many surgeons are experienced. As anywhere, safety depends on the surgeon, the facility and good follow up. The bigger risk with this procedure is choosing it when it does not suit your face.

Will I save money compared with the UK?

Usually yes. Buccal fat removal in Czechia is commonly €1,200 to €2,200, against roughly £2,000 to £5,000 privately in the UK. Weigh the saving against travelling for follow up. Reviewed June 2026.

Does the fat grow back?

No. The buccal fat pads do not return once removed, which is why a conservative approach matters. The face also loses volume naturally with age, so over removal can look worse over time.

How soon can I fly home?

Many people fly after a few days, but early checks matter and swelling is still settling. Plan your stay around the review schedule rather than the cheapest flight.

What if I do not like the result?

Removed fat cannot be put back, so judge it slowly over months. If you are unhappy, options are limited, which is why a cautious surgeon and clear expectations beforehand matter most.
