The real experience

Recovery, and the follow up.

Buccal fat removal, sometimes called a bichectomy, takes part of the buccal fat pad out of each lower cheek to slim the mid face. The surgeon works through a small incision inside the mouth, so there is no external scar, and the operation is usually short and done under local anaesthetic with sedation. Most people stay in Portugal for a few days so the surgeon can review healing before they fly.

Recovery centres on the mouth. Swelling makes the cheeks look fuller at first, which can be alarming because it is the opposite of the goal, and the slimmer result only appears as the swelling settles over weeks to a few months. A soft diet, careful mouth rinsing, and good oral hygiene reduce the infection risk from incisions inside the mouth. Your surgeon will tell you when it is safe to fly and how to care for the incisions.

The follow up is where medical travel gets harder, and it matters more here because the result is permanent. Asymmetry, an infection, or a feeling that too much was removed is awkward to manage from another country once you are home, and adding volume back is far harder than taking it away. Agree before you travel how reviews are handled, and line up a local doctor for urgent care. Standard travel insurance rarely covers elective cosmetic surgery or its complications.

This looks like a minor procedure but it sits near important structures and it cannot be undone. The risks are real, and travelling for it adds its own layer.

Removing too much, and premature ageing

The biggest regret is over removal. Because the face loses volume naturally with age, taking too much fat can leave the cheeks hollow and the face looking older in later years. Adding volume back is difficult.

Facial nerve injury

Branches of the facial nerve run close to the buccal fat pad. Injury is uncommon but can cause weakness or altered movement of the cheek, sometimes lasting.

Injury to the salivary duct

The duct that drains a salivary gland passes through the area. Damage to it is rare but troublesome, and it is a reason the operation should only be done by a surgeon who knows the anatomy well.

Infection and asymmetry

Incisions inside the mouth carry an infection risk, and uneven removal can leave the cheeks asymmetric. Correcting asymmetry is difficult because fat cannot simply be put back.

The travelling risk

A complication or regret after you land is the central problem of going abroad, and it bites harder when the result is permanent. Local surgeons are often reluctant to manage another clinic's work. Settle the aftercare terms in writing first.
